SeniorLIFE provides or arranges for a full array of health and social services, 24 hours a day, 7 days a week, 365 days a year. As part of the SeniorLIFE enrollment process, a team of geriatric doctors, nurses, social workers, rehabilitation specialists and other health care professionals will assess the member's needs.
Our goal is to maximize independence and improve the quality of life for all our members. The team then creates an individualized care plan with you, your family or caregiver in mind. As medical or social needs change, the care plan is modified accordingly.
